Employee Card Proposal Requirements

Requirements:

  1. Civil Servant Candidate Decree (legalized)

  2. Civil Servant Decree (legalized)

  3. Last Rank Decree (legalized)

  4. 2 Latest Photos

Requirements for Wife and Husband Cards

Requirements:

  1. First marriage list

  2. Copy of marriage book/marriage certificate

  3. Family structure list

  4. Spouse's photo pass 2 Sheets

Service Flow

  1. The applicant prepares all the requirements for applying for Karpeg, Karis and Karsu as required to the Head of the Work Unit.

  2. Head of work unit sends to Universitas Sumatera Utara Human Resources Bureau

  3. HR Bureau verifies and processes cover letters proposing employee cards (Karpeg), wife cards (Karis) and husband cards (Karsu).

  4. HR Bureau uploads all required documents on the system https://layanan-sdm.kemdikbud.go.id
