Requirements to become a BPJS participant at the Universitas Sumatera Utara
Requirements :

Attach a decree of appointment as permanent / non-permanent non-civil servant education personnel

Attach a photocopy of the family card
-
Attach a photocopy of the identity card of the person concerned / husband/wife (if married)
-
Active email
-
Active cell phone number
-
Active BPJS health and employment cards (if existing); if not, make a new one
-
Marriage book (if married)
-
Childbirth certificate (if married and have children)

Attach the Rector's Decree Regarding Reactivation in the Functional Position of Lecturer

Attach Teaching Decree from Dean of Faculty
